 Students who are new to the Clayton County Public School System will need to provide records from their previous school.  Upon enrollment, please be able to provide the previous school’s name, address, and phone and fax number.
An official transcript from the previous school is required at the time of enrollment to insure proper grade placement.

(NO EXCEPTIONS* Documents must be readable)

Original Birth Certificate, with Parent name - REQUIRED

Student’s Social Security Card - REQUIRED

Immunization Record (GA3131) - Must be on GA FORM

Eye, Ear, Dental Record (GA3300) - Must be on GA FORM

Parent/Guardian Photo Identification - REQUIRED

Proof of Guardianship - REQUIRED

Grade Placement/ Withdrawal Form - REQUIRED

Discipline Record – REQUIRED

Two Proofs of Residency - REQUIRED

 

All must be in the parent/guardian’s name and dated within the past two months.

    Withdrawals will be completed in the counseling suite by our registrar, Ms. Veronica Miller.  All withdrawals will be completed within 24 hours if submitted before 2:00 p.m. Monday – Friday.  All requests submitted after 2:00 p.m. will be completed within 2 business days.  Completed withdrawals can be picked up in the counseling suite by 3:00 p.m. daily.
