REGISTRATION INFORMATION

Welcome to Jonesboro High School.  In order to enroll your son/ daughter it will be necessary for you, as the biological parent or legal guardian, to accompany him/her upon registration. 

It is  necessary for you to provide the following information:

1.   TWO PROOFS OF RESIDENCE.           
Current utility bill           
Copy of home mortgage payment book    
Apartment lease showing name of parent/legal guardian
Receipt to have utilities connected           
Driver’s license showing parent/legal guardian’s current address
Notarized Affidavit of Residency with two of the above

2.  VERIFICATION OF GRADE PLACEMENT. 

This may include a withdrawal form from the previous school, an unofficial transcript, or final report card.

3.  DISCIPLINE INFORMATION. 

A disciplinary record from the Georgia school previously attended for 7^th grade and above.

4.  SOCIAL SECURITY CARD.

5.  VERIFICATION OF GUARDIANSHIP. 

Appropriate records for verification of guardianship are as follows:  divorce decree granting joint or sole custody; temporary order for placement through Probate Court; placement documents issued through Juvenile Court and/or DFACS.  When parents have never been married, the birth mother is the legal guardian and is verified by the certified birth certificate.  Fathers of children born out of wedlock must provide verification of custody (legitimation) obtained through Superior Court.

6. GEORGIA IMMUNIZATION (Form 3231) and Georgia Eye, Ear, and Dental Certificate (Form 3300) are required for all students.*

*Out-of-state immunization documents must be transferred to the appropriate Georgia forms.  All immunization documents can be obtained from either your private physician or from any local health department.  The closest offices are listed below:

7. PROOF OF BIRTH. 

A certified copy of the student’s birth certificate or other legal document showing birth date and names of biological parents.
